GENERAL DESCRIPTION
Within the context of the vision, mission, and strategic direction of Kinonje Abinoonjiiag Niigan Inc., and under the direction of the supervisor. The Case-Aide is responsible for providing a support function to case managers with respect to families and children in care, such as following a case plan that the case manager completed for the family/children in care.
The case-aide will assist Case Managers to develop and pursue their goals and will support them to identify and access services/resources relevant to their specific needs. The Case-Aide will support families to connect with existing supports and resources within their communities including housing supports, education resources, employment services/training, life skills training (e.g., financial management, household managements). The Case-Aide help families involved with Child Welfare Services and/or children in care clients that are supportive. Case-Aides undertake a variety of social services support responsibilities that are complementary to the role of a case worker.
RESPONSIBILITIES AND DUTIES
Responsibilities:
SUPPORTS To Case Manager
Under the direction of the Supervisor and Case Manager, the Case-Aide worker is responsible for:
- Working effectivley with Case Manager to assist with problematic family situations.
- Assisting Case Manager with transportation of children and/or families when required.
- Asssiting with observing and documenting family visits when needed.
- Help family members understand what is expected of them during visits to ensure that visits are child focused, meaningful, purposeful, and contributing to the family’s case plan.
- Responsible for submitting all documentation for interactions with clients in a timely manner.
- Document observations, concerns, and/or conversations for the file and follow up with Case Manager.
- Flex your working hours to your scheduled transportation and/or visits.
- When using agency vehicles and visiting rooms, tidy/clean up after each use.
- Implement safety plans where necessary and effectively intervene during a visit if a child’s safety or well-being appears to be at risk.
Specific duties:
- Provide support to Case Managers with case management responsibilities.
- Support and empower children and families in their reaching goals.
- Provide support with events and special projects by assisting with preparations prior, during and after Agency activities.
- Develop and maintain an accurate work calendar.
- Ensure confidentiality and safekeeping of all Agency documents and recordings.
- Assist transitional youth in care through provision of individual support relative to attending appointments and access with family and community.
- Inform supervisors and Case Managers of family porgress, changes, and/or case updates that may impact children’s well being.
- Provide individual support to families in their home with parent coaching and support service relative to the current service plan and collaberation of service supports.
- Other case management duties as assigned.
QUALIFICATIONS
- Must have a valid Manitoba driver’s licence, a registered working vehicle, and may have to submit drivers abstract.
- Ability to work effectively independently and within a team.
- Must always maintain confidentiality of agency and client information.
- Must demonstrate problem-solving, planning, priority-setting, and conflict resolution skills.
- Ability to work in a fast-paced work environment and to deal with stressful situations.
- Must be willing to provide a criminal record check with vulnerable persons check, child abuse registry check, and a prior record check.
- Must be willing to work flexible hours, and weekends.
